The solution, now built on Servelec’s Conexes interoperability platform, and available to other Mosaic users, was designed and developed by Nottinghamshire County Council to support the hospital discharge process. Enabling direct system to system messaging and updates between the Mosaic social care case management system, also provided by Servelec, and the electronic patient records (EPR) of the county’s acute healthcare trusts. Nottinghamshire County Council have shared this process development and information standard to be to be available at a national level.
The development provides staff at the Council with an up-to-date view of patient progress and history, making the referral process both quicker and safer, as there is less room for error or missed information. Having all the correct information in one place saves staff time and allows patients to be discharged sooner into social care support where needed. This has been more vital than ever during the Covid pandemic.
The Discharge to Assess solution is one component of a wider project established by Nottinghamshire County Council, which has been a customer of Servelec since 2014, to improve health and social care integration in the County. In November 2019, the Council secured funding from NHS Digital, as part of the NHS Pathfinders programme, to scale up and roll out its innovative interoperability solutions to other organisations; Servelec will be working in partnership with the Council to provide this.

Discharge to Assess has been working at all Hospital Trusts in Nottinghamshire since July 2020, and it will be available to other councils and trusts via the Conexes platform from April 2021.
